Timely Responses to Record Requests

We want to remind you and your office staff of the importance of promptly completing record requests. Doing so is vital for providing your patients with exceptional dental care.

All record requests you receive will come from our Clinical Reviews department email address, clinicalreviews@lsvusa.com.

Please respond to these requests within five business days. This is pursuant to sections 3.10 and 9.2 of our Participating Provider Agreement and Florida Statute 466.028(1)(n), which specifies that you are required by law to provide this information on time and completely. Delaying the completion of record requests may be in violation of this Florida statute.
