Dental Records Request

As part of our Quality Assurance and Utilization Management program, we conduct periodic audits of dental records to assess the delivery of necessary and appropriate care. As a result, you may receive an encrypted email from our clinical operations team requesting specific patient records. It will be sent from clinicalreviews@lsvusa.com. These requests may also be related to appeals or grievances.

These emails include important information that is time sensitive, so please be sure to check your inbox and respond promptly to these inquiries.
